Free HRT in Ireland: how the scheme works — and how to walk in prepared.
Since 1 June 2025, HRT prescribed for menopause can be free at participating pharmacies if you have a medical card or a Drugs Payment Scheme card. That is a State scheme. It is not a diagnosis, and it is not us telling you to start or stop anything. Here is the map so a ten-minute GP visit is not wasted on paperwork you could have done on Tuesday.
What changed
For years, Irish women paid for HRT out of pocket or against the Drugs Payment Scheme monthly cap. From 1 June 2025 the State meets the cost of HRT products prescribed to alleviate menopause symptoms, including the pharmacy dispensing fee, at pharmacies that have joined the scheme. Citizens Information and the HSE both describe the same two keys: a valid prescription, and either a medical card or a Drugs Payment Scheme (DPS) card.
If you do not have a medical card, you can apply for a DPS card if you are ordinarily resident in Ireland. There is no separate “menopause means-test” for the HRT arrangement itself. You still need a clinician to decide whether HRT is suitable for you.
The three things that actually matter
The scheme does not skip the medical decision. Bring a picture of your week, not a panic. The Menopause Passport and ten-minute GP page exist for that clock.
Apply for the Drugs Payment Scheme at HSE.ie if you need it. Full name, date of birth, PPS number, contact details. Do this before the appointment if you can — not after, in the car park.

What HRT is, in one honest paragraph
UK NICE guideline NG23 (updated November 2024, reviewed April 2026) is clear: offer HRT first-line for vasomotor symptoms associated with menopause — hot flushes and night sweats — unless it is unsuitable. Menopause-specific CBT can sit alongside it, not as a replacement for people who want hormones. NICE also says HRT does not affect life expectancy, and that talking therapy should not be sold as “instead of HRT” for everyone. The International Menopause Society (December 2025) says menopausal hormone therapy is the most effective treatment for bothersome vasomotor symptoms when there are no significant contraindications.
That is the medical default in the evidence. It is still your conversation with a doctor who knows your history. We will not tell you to take it. We will not tell you to refuse it. We will help you arrive with a week you can describe.
If HRT is not for you
Some women cannot take it, some will not, some are still deciding. NICE now includes a non-hormonal option — fezolinetant — for moderate to severe vasomotor symptoms when HRT is unsuitable (technology appraisal TA1143, 2026). That is a specialist/GP conversation, not a wellness claim. Older non-hormonal medicines were never first-line for flushes alone. Lifestyle — sleep, protein, strength, alcohol, load — still belongs to you either way. That is the Method. It does not replace a prescription you and your doctor choose.
What a ten-minute Irish GP visit is actually for
It is not for your life story. It is for: what has been loud this month; how long; what you have already tried; whether other causes should be ruled out (thyroid, iron, and more); and whether you want to discuss treatment options including HRT. If you are under 45, say your age in the first sentence — early and premature menopause are handled differently. If bleeding is unusual (very heavy, between periods, after sex, or after 12 months without a period), that is a medical evaluation, not a “wait and see”.
HSE pages also note: a review is usual around three months after starting HRT, then yearly. Specialist complex menopause clinics exist for the cases GPs refer. You do not walk into those without a GP.
The subtle trap
Free HRT can feel like the State has “solved menopause.” It has not. A scheme removes a cost barrier for people who already have a prescription. It does not give you a longer appointment, a menopause-trained GP, a workplace that can hear the word, or a week you can live. Those are still your work, and ours.
